NOTICE OF PRIVACY AND PROCESSING OF PERSONAL DATA
   
 

THE "JESUS EUCARISTIA" SCHOOL, Educational Institution, existing and duly constituted in accordance with the laws of the Republic of Colombia, recognized by Resolution 001815 of December 18, 1981, issued by the Secretary of Education of Boyacá, with address in the city of Duitama -Boyacá, and identified with the NIT 23541925-2 and Code ICFES 057133; in accordance with the provisions of Statutory Law 1581 of 2012 and Regulatory Decree 1377 of 2013, establishes the following internal policy for the adequate Treatment and Management of Personal Data that have been collected and stored in their databases; for this reason, issues the present policy of treatment of personal information, in compliance with the Constitution and the Law, in accordance with the various processes that are handled with students, parents, employees, suppliers and visitors of the institution, where THE SCHOOL, holds the quality of Responsible for the treatment of personal data.
In order to maintain this custody, to carry out any consultation or issue of information related to the data handled by the institution, the following procedure must be followed:

  1. Manage at the school orDOWNLOAD HERE the form for Request of Personal Information that is being handled by the institution. If you want to apply for certificates then you must manage in the school orDOWNLOAD HERE the Certificate Request Form (Registration, Notes, Labor, Proceedings of Grado, copies of Diploma, etc).
  2. The form must be filled out by the Parents or Persons in Charge who have authorized the School to process Information and present it on the campus, attaching a copy of the identity document to the School.
  3. In the case of certificates if there is any shipping cost in the institution, make the corresponding payment.

Once the application procedures have been completed, the School will proceed with the evaluation of said request and will proceed in accordance with the provisions of the Institutional Policy on Information Processing for Personal Information Requests. For the issuance of certificates, the deadline is one or two business days, except for copies of the Diploma of Degree that require two weeks.

UPDATING OF DATA AND / OR CLAIMS
   

If it is required is to update the data handled by the institution or make a claim about the treatment of them, the following procedure is followed:

  1. Manage at the school or DOWNLOAD HERE the form for Update / Claim
  2. The form must be filled out by the Parents or Persons in Charge who have authorized the School to process Information and present it on the campus, attaching a copy of the identity document to the School.

Once the application procedures have been completed, the School will proceed with the evaluation of said request and will proceed in accordance with the provisions of the Institutional Policy on Information Processing.
